Forensic evaluations. Not covered by insurance. Payment upon booking appointment is required. All paperwork must be completed and submitted 24 hours prior to your appointment date. Reschedules must be made 48 hours prior to appointment or the full fee is charged.

30 min - Basic Diagnostic Evaluation - Virtual $150 total.
You will receive an official diagnostic letter (when indicated) for your prescriber's file.
You must pay $50 deposit at time of booking appointment that is non-refundable. Balance of $100 is charged at end of appointment.
